5 Things I Would Do for an Acute Flare Up of Lower Back Pain: A Physio's Guide

Acute lower back pain can strike out of nowhere; one wrong move lifting the groceries, a long drive, or simply waking up stiff and sore. It's one of the most common reasons we see people in clinic, and the good news is that most episodes resolve quickly with the right approach. 


Here are five things to do in the first few days to give your recovery the best possible start.


1. Keep Moving (Gently)


It might feel counterintuitive, but rest is rarely the answer for acute lower back pain. Research consistently shows that staying as mobile as possible leads to faster recovery than bed rest alone. You don't need to push through sharp pain, but gentle walking, light stretching, and continuing your normal daily activities as much as possible will help build confidence in movement and prevent your back from stiffening up further.


Try: short walks of whatever you can tolerate, a few times a day.



2. Avoid Positions That Aggravate Things Further


Pay attention to what positions or postures make your pain worse and modify your environment accordingly. Common culprits include sitting on a low, soft couch for long periods, slouching over a desk, or staying in any one position for too long.


This doesn't mean avoiding these positions forever, just while your back is acutely flared up. The key is variety and not letting yourself get stuck.


Try: rotate between sitting, standing, lying down, and walking around throughout the day rather than staying in one position.



3. Take Pain Relief If You Need It


There's no benefit to suffering through the pain. If over-the-counter options like paracetamol or anti-inflammatories help you stay mobile and sleep comfortably, they're a perfectly reasonable short-term tool. The goal here is to take the edge off so you can keep moving and functioning which will help in the long run. 


Always check with your pharmacist or GP if you're unsure whether a medication is appropriate for you.


4. Hydrate, Eat Well, and Limit Alcohol


Hydration plays a bigger role in back health than most people realise. Your muscles, tendons, and connective tissue all depend on adequate fluid to function well and recover efficiently. When you're dehydrated, muscles are more prone to cramping and fatigue, and your body's ability to repair tissue slows down.


What you eat matters too. Processed foods, refined sugars, and alcohol can all drive systemic inflammation, which amplifies pain signals throughout the body. Focusing on whole foods and limiting alcohol consumption early on helps create the best internal environment for the body to heal. 


5. See a Physiotherapist Early


The sooner you get an accurate assessment, the sooner you'll understand what's going on and have a clear plan to fix it. A physiotherapist can identify the cause of your pain, rule out anything more serious, and provide specific exercises and hands-on treatment tailored to your condition. Once the acute flare up has settled, we can also take a broader look at why it occurred in the first place and help make sure it doesn't keep coming back.


Early intervention is consistently linked with faster recovery and a lower likelihood of back pain becoming a chronic, ongoing issue. Don't wait weeks hoping it will resolve on its own, most acute lower back pain responds well to physiotherapy, often within just a few sessions.

While most acute lower back pain is mechanical and not dangerous, please see a doctor or go to emergency if you experience any of the following:

  • Loss of bladder or bowel control

  • Numbness or tingling in the groin or inner thighs

  • Severe pain that is constant, worsening, and unrelated to movement

  • Pain following a significant fall or accident
